about your first visit

First, you will call us to make an appointment.　（Time required：app.5~10min.）  TEL: 03-5225-1291　
   We prioritize patients with appointments. We may not take you when you visit us without an appointment.
        On the call, we may ask you name, phone number, type of insurance, and current/past treatments in other clinics.

Please visit us on time on your appointment day, and present your insurance/ ID cards at the reception desk. 　
You will tell a receptionist that it is your first visit, and provide your insurance/ ID cards.
We will make a photocopy of your insurance/ ID cards, and return them to you when you make a payment.

Please fill out some medical questionaires.　（Timre required：app.15~30min.）
Before the consultation with your doctor, you will fill your information on questionnaires.
You may fill your information as detailed as possible. If you don’t know how to write, you are more than welcome to ask questions at the reception desk.

INTAKE　（Time required：app. 30min.）
You may have an intake session with a psychologist before seeing your doctor.
Before your medical consultation, a psychologist may gather necessary information and report it to your doctor.

Medical consultation　（Time required：app. 30~45min.）
You will see your doctor.
Your doctor will listen to your concerns and will decide your treatment plans.

Payment　（Time required：app. 5min.）
When you finish your medical consultation, please wait again at the reception desk.
When your name is called, you will make a payment and receive your receipt and prescription (when your doctor prescribe medications).
